Winneshiek County healthcare providers ask that county residents who have been vaccinated outside of Iowa to either call their medical provider . . .

Winneshiek County healthcare providers ask that county residents who have been vaccinated outside of Iowa to either call their medical provider or place information in their MyChart record for yourself with Decorah Medical Clinic or Gundersen Health System. Currently, there is no federal data base that allows transfer of that information.
Individuals should provide the following information: type of vaccine received; dates of vaccination; location where the vaccine clinic was held; name; birthdate.
Residents of Winneshiek County who do not have a medical provider locally and received a vaccine out of state should contact Winneshiek County Public Health at (563) 382-4662.
